Smoking is a leading preventable cause of disease and death, contributing to cancer, COPD, and cardiovascular disease. Use the 5 A’s framework — Ask, Advise, Assess, Assist, Arrange follow-up — and offer nicotine replacement, which raises quit rates by roughly 50–60%. Quitting at any age yields measurable health benefits within weeks, so it is never too late to counsel cessation.

Obesity

Excess body fat, typically BMI ≥30, that increases risk of chronic disease.

Obesity is excess body fat, generally defined as a BMI of 30 or higher, that raises the risk of diabetes, hypertension, heart disease, and many cancers. Even a 5–10% weight loss meaningfully improves blood pressure, glucose, and lipid profiles, so modest goals are worthwhile. Effective management combines diet, activity, and behavior change, with pharmacotherapy or bariatric surgery added when appropriate.

Mammogram

X-ray screening of breast tissue to detect cancer early.

A mammogram is an X-ray screening of breast tissue used to detect cancer before it is palpable. The recommended starting age varies by guideline and individual risk, generally between 40 and 50, so follow the local recommendation. Teach clients to avoid deodorant, lotion, or powder on the day of the exam because these can appear as calcifications and obscure the image.

Literacy

Health literacy is the ability to understand and use health information.

Health literacy is the client’s ability to obtain, understand, and act on health information. Use plain language and visuals, and confirm understanding with the teach-back method — asking the client to explain it back — rather than the closed question “do you understand?” Low health literacy is common and is a major driver of poor outcomes, medication errors, and readmission.
