Vitals

Monitoring is ongoing, focused assessment aimed at detecting complications before they become emergencies. Know the critical lab values — such as potassium below 2.5 or above 6.5 mEq/L — that require immediate notification rather than routine charting. Document trends over time, since the direction of change is often more telling than a single value, and report deviations to the RN promptly.

Specimen

Blood, urine, sputum or wound drainage sent for testing; label at the bedside and collect cultures before antibiotics start.

A specimen is any sample sent for laboratory analysis: blood, urine, stool, sputum, or wound drainage. Collecting them correctly is core LPN work, and most of the exam questions turn on timing, container, and labelling rather than on the test itself.

Collect cultures before the first antibiotic dose, or the drug may suppress the organism and the result comes back falsely negative. Use the container the test requires, since the preservative or additive is part of the test. Label at the bedside in front of the client, using two identifiers, rather than labelling in the hallway afterwards; mislabelled specimens are a common and preventable source of serious error. Some samples have their own rules worth memorising: a clean-catch urine needs perineal cleansing first, a sputum culture is best on waking, and a stool sample for ova and parasites must reach the lab while still warm. Report a delay or a spoiled sample to the RN rather than sending it anyway.

Urinalysis examines urine for color, clarity, specific gravity, and the presence of cells, bacteria, glucose, or protein. A clean-catch midstream sample reduces contamination, and the specimen should be refrigerated or processed within about an hour to keep results accurate. Cloudy, foul-smelling urine with fever in an older adult suggests a urinary tract infection and possible delirium.

Glucometer

A bedside device that measures capillary blood glucose.

A glucometer is a bedside device that measures capillary blood glucose from a fingerstick sample. Wipe away the first drop of blood and sample from the side of the fingertip to reduce pain and improve accuracy. Follow the “rule of 15” for hypoglycemia — give 15 g of fast-acting carbohydrate, then recheck the glucose in 15 minutes.

Telemetry

Continuous heart-rhythm monitoring from a central station; the LPN keeps the leads attached and reports every alarm.

Telemetry is continuous remote monitoring of a client’s cardiac rhythm, allowing the team to catch dangerous changes in real time. Always assess the client, not just the monitor — confirm an apparent lethal rhythm by checking the actual pulse and responsiveness, since a “flatline” is often a loose lead. Correct lead placement and reducing artifact ensure an accurate, trustworthy tracing.

X-ray

A diagnostic image that uses ionizing radiation to show bones and dense structures.

An X-ray uses ionizing radiation to produce images of bones and other dense structures, useful for fractures, pneumonia, and tube placement. Ask women of childbearing age about possible pregnancy before any X-ray and shield the abdomen and reproductive organs as appropriate. Remove metal objects from the imaging field, since they obscure the picture.

MRI

Magnetic resonance imaging — a powerful magnet that produces detailed soft-tissue images.

MRI (magnetic resonance imaging) uses a powerful magnetic field to produce detailed images of soft tissue, with no ionizing radiation. Screen carefully for all metal — pacemakers, aneurysm clips, cochlear implants, and embedded shrapnel — because the magnet can dislodge or heat metal, a life-threatening hazard. Claustrophobic clients may need pre-medication or an open MRI to tolerate the enclosed scanner.
