Elimination covers the excretion of waste through urine and stool, and tracking the client’s normal patterns and intake/output helps detect problems early. Report significant changes such as low urine output, blood, or a sudden shift in bowel habits. Promote regularity with adequate fluids, fiber, activity, and privacy.
